Nelson City Medical Centre – Nelson & Titoki

Nelson City Medical Centre is a busy general practice with a range of patients representative of the city.  We operate as one entity, but we have two physical locations – Nelson on Collingwood Street and Titoki, three minutes drive away on Waimea Road. We are 100% owned by South Link Health Services, under the umbrella of the South Link Education Trust.

We currently have 8 GP’s, a Nurse Prescriber, 5 Nurses, 4 Admins/Receptionists, a Practice Manager and a Second-in-Charge.

We are part of Nelson Bays Primary Health, who are our PHO. 

We have a great team culture.  We work hard to provide our patients with the best possible care by providing a holistic approach to their healthcare needs.  We strive to help all our patients live happy and healthy lives and we really believe in what we do.  Both our Practices are located in central Nelson, parking is easy, we also have bike racks available if you would rather bike to work on the many sunny days that we have in Nelson.  There are Pharmacies and other health professionals in close proximity to both Practices.

We use the latest version of cloud-based Medtech Evolution as our Patient Management System, and we have Manage My Health, so patients can look at their results, book appointments and order repeat prescriptions.  We have a Dunedin-based South Link Health IT team who are supportive and knowledgeable and assist whenever we encounter issues. We have strong collegial relations with primary and secondary services, as well as pharmacy and allied health.  Both Practices are Foundation-accredited. 

Combined we have 5600 patients, of which 10% identify as Māori, 7% Asian, 2.5% Pasifica and 10% Quintile 5.  We are proactive in ensuring equity.

We are open Monday to Friday and operate morning and afternoon sessions. Each session has twelve 15-minute patient appointments.  

We conduct full inductions onsite for all new staff members.  We provide ongoing training to keep up to date with regulations. We have weekly peer review, and monthly full team meetings.

We have mandatory training for Privacy, Child Safety, Cultural competency, and Te Tiriti o Waitangi, via online training.

We have many resources available for all staff to access via our South Link Health Teams platform – Policies and Procedures, Te Reo Māori, Tikanga Māori, Guidelines for Foundation and Cornerstone accreditation.
